    Iraqi forces advance near Mosul as IS attacks western town

    KHAZER, Iraq (AP) — In a story Oct. 23 about Iraq’s Mosul offensive, The Associated Press reported erroneously that a UNICEF team had visited a refugee camp near Mosul and found poor conditions there. The people the UNICEF team visited were residents of the area, which had recently been retaken from the Islamic State group.
